It will be your new GTA account I believe post your appeal as normal putting your original ID for where it asks for a perm id, include the picture of the ban message with the new ID and explain the situation within your appeal.

You know what you did hence your ban. If you think it’s wrong make a unban appeal.

[I appealed my ban and got accepted but I got my old CFX account back and would like to use that instead now however i am banned again.]

[Discord role issue]

To get your discord roles all you need to do is open the cmg discord and go to #tag-request and put what role you need and someone in staff will ask for a tebex receipt number, send them a screenshot of this and they will add your roles.
